Community Engagement VISTA Leader

(1 Position)

The AmeriCorps VISTA Leader will be the point of contact in coordinating the Community Engagement VISTAs in the following areas. The VISTAs will establish a formal process for intake and delivery of community outreach requests. They will produce health education materials and develop the organizational infrastructure to record, measure and review these engagement activities. The activities will focus on benefiting the low-income and medically uninsured community within Memphis. The VISTA Community Engagement Corps also seeks to promote interest and support for Church Health services. Engagement activities will include health education presentations, promotion of medical/behavioral health/wellness services and recruit volunteers and supporters for the organization. This project aligns with the Healthy Futures focus area. The eight Community Engagement Corps VISTAs will be an integral part of multiple service areas within the organization to gain expertise in the department services

Communications Coordinator

(1 Position)

The VISTA Church Health Communications Coordinator will be an integrated member of the Communications team to create, implement and evaluate new, sustainable communication campaigns to improve the department’s overall effectiveness in connecting with and serving Church Health patients, donors and partners within community. The Coordinator will be addressing issues of poverty by increasing Church Health’s capacity to reach new audiences to receive services and support our organization’s ability to serve the health and wellness needs of more uninsured community members.

Development Coordinator

(1 Position)

The VISTA Church Health Development Coordinator will serve as an integrated member of the Church Health Development team by strengthening and contributing to the donor cultivation, management and stewardship strategy. The VISTA Development Coordinator will be addressing issues of poverty by increasing Church Health’s financial and donor relations capacity to meet the growing healthcare needs of the working uninsured population within Memphis and Shelby County.

Faith Community Engagement Coordinator

(1 Position)

The VISTA Church Health Neighborhood Faith Communities Coordinator will help build Church Health’s capacity to serve faith communities through the creation of a sustainable health promotion training program utilized by Memphis congregations to increase the health literacy and reduce illness within their congregational members in low income areas. The VISTA Coordinator will be address issues of poverty by partnering with and empowering 5 faith communities to address the health issues that impact their members and community. The success of this initiative will be the template for ongoing expansion throughout the Memphis faith community.

MEMPHIS Plan Coordinator

(1 Position)

The VISTA Church Health MEMPHIS Plan Coordinator will serve Memphis/Shelby County population in need of medical and wellness services. Areas of emphasis will be on promoting medical services, delivering educational outreach and building Church Health’s capacity to serve more community members. The VISTA will be addressing issues of poverty by connecting those in need of healthcare services to the necessary medical resources. The VISTA will develop sustainable data collection processes to measure the customer service needs of our medical volunteers and business partners and collaborate with them to increase Church Health’s capacity to serve more working uninsured.

Nutrition Hub Coordinator

(1 Position)

The VISTA Church Health Nutrition Hub Coordinator will focus on developing new strategies and build Church Health’s capacity to increase patient engagement and participation in wellness programming. The VISTA will also serve the Memphis community through improved access to nutrition education. The VISTA will address issues of poverty through improved health literacy for our low-income population, pertaining to improved patient nutrition behaviors, increasing healthy food access and reduction of food disparities/chronic illness across economic strata.

Opioid and Behavioral Health Outreach Coordinator

(1 Position)

The VISTA Church Health Behavioral Health and Opioid Outreach Coordinator will serve Church Health patients and the Memphis/Shelby County population in need of behavioral health and substance abuse prevention services. Areas of emphasis will include opioid abuse prevention and harm reduction, drug/alcohol/tobacco treatment, depression and anxiety. The VISTA Coordinator will be addressing issues of poverty through the development of sustainable behavioral health and addiction programming that acknowledges the health literacy of the Memphis and Shelby county community.

Outreach and Engagement Coordinator

(1 Position)

The VISTA Church Health Outreach & Engagement Coordinator will serve Memphis/Shelby County population in need of medical and wellness services. Areas of emphasis will be on delivering educational outreach and building Church Health’s capacity to serve more community members. The VISTA will be addressing issues of poverty through development of sustainable outreach programming that acknowledges health literacy, connecting those in need with necessary resources to achieve maximum health and building community partnerships to enable Church Health to expand ability to meet community needs.

Physical Rehabilitation Outreach Coordinator

(1 Position)

The VISTA Church Health Physical Rehabilitation Outreach Coordinator will build the capacity of Church Health’s Physical Rehabilitation Department to meet the rehabilitation needs of uninsured Memphians and reduce the disparities and services available to them. The VISTA will help the uninsured population reduce poverty through improved access to physical rehabilitation, pain management and improving patients’ ability to return to work, enhancing their overall quality of life.

Well Coordinator, Child and Family Outreach

(1 Position)

The VISTA Church Health Well Outreach Coordinator will serve Memphis/Shelby County children and family populations in need of wellness intervention services. Areas of emphasis will be on delivering educational outreach, wellness programming and building Church Health Well’s capacity to serve more community members. The VISTA will be addressing issues of poverty through development of sustainable family outreach programming that acknowledges health literacy, connecting those in need with necessary resources to achieve maximum health and building community partnerships to enable Church Health to expand ability to meet community needs.

Workforce Engagement Coordinator

(1 Position)

The VISTA Church Health Workforce Engagement Coordinator will serve Memphis/Shelby County uninsured population in need of medical and wellness services. Areas of emphasis will be on partnering with the Workforce Development and Human Resources teams to create new pathways to learning for staff, patients and community partners. The Coordinator will be addressing issues of poverty by improving the organization’s ability to positively impact health literacy and capacity to meet the healthcare needs of the working uninsured within the Memphis Community. This will be accomplished by creating a sustainable library of training materials to support staff in learning how to best engage with each other, our patients, community partners.
