Obtaining Your Records
As an established patient, some of your records, such as your current medication list or visit summaries, can be found on your Patient Portal. Otherwise, you must come in person to fill out and sign a release of records form stating that you give permission to send out records. Once this form is completed, most records requests are usually handled within 7-10 business days.

If you are an established patient and purchase a plan from the Affordable Care Act marketplace exchange, Church Health currently accepts the following plans:
