Behavioral Health Services
The Church Health Social Services team provides established patients referrals to community resources as well as assists them with working through complex issues.
Our social workers can direct patients to agencies in the community that provide rent, mortgage and utility assistance, housing, food resources, medical supplies and equipment, transportation, and other resources.
Social services are available to Church Health patients only. Please contact your provider for more information.
Free Child Care During Your Behavioral Health Appointment
If you have children between the ages of 6 weeks and 12 years, we encourage you to check them into Church Health’s children’s program, The Well, located on the first floor. This is a service provided to you free of charge during your scheduled appointment time.
