Name-brand medications available on the Patient Assistance Program are ordered by Church Health staff with patient providing necessary income documentation, photo identification, and signatures. Patients’ income determines eligibility per each pharmaceutical company limits.
Depending on the directive of each pharmaceutical company, medications on the Patient Assistance Program are either:
– Shipped directly to patient’s home at no charge, or
– Picked up at the Outpatient Pharmacy at Methodist University Hospital for a $6 dispensing fee.
Patient Assistance Medications are delivered in either 90-day or 120-day supplies, per the discretion of pharmaceutical company.
