Behavioral Health Services Overview

At Church Health, we recognize that your spiritual and emotional well-being plays an important role in your physical health. Our on-site staff and network of volunteer licensed mental health professionals can provide you with the emotional support you need to work through issues such as depression, anxiety, stress, grief, and substance use issues. Our Social Worker assists in addressing the social needs of our established patients, connecting patients to community resources such as food resources, housing, financial assistance and more. 

Counseling Services

Traditional counseling is available, by appointment, to uninsured established Church Health patients and MEMPHIS Plan participants only. Learn more about how to become as established patient here. Services are based on a sliding scale based on income.

Where Is Church Health Behavioral Health Located?

Parking is available along Concourse Avenue and in the Crosstown Concourse parking garage. Please enter through the West Atrium entrance on the west side of the building, where you’ll also find drive-through access and a convenient drop-off area. Once inside, take the elevator in the back of the West Atrium, past the Church Health Welcome Center, to the third floor. A receptionist will greet you and guide you to the Behavioral Health waiting room for check-in.

Social Services

The Church Health Social Services team provides established patients referrals to community resources as well as assists them with working through complex issues.

Our social workers can direct patients to agencies in the community that provide rent, mortgage and utility assistance, housing, food resources, medical supplies and equipment, transportation and other resources.

Social services are available to Church Health patients only. Please contact your provider for more information.

Free Childcare During Your Appointment

If you have children between the ages of 6 weeks and 12 years, we encourage you to check them into Church Health’s children’s program, The Well, located on the first floor. This is a service provided to you free of charge during your scheduled appointment time.

8:00 a.m.–12:45 p.m.
2:00 p.m.–8:00 p.m.

Note: The Well has a two-hour time limit for each child. Please arrive early for your appointment to transition your child downstairs to sign in. Alert the front desk staff that your child is checked into our children’s program.
