Patient Eligibility
Church Health Physical Therapy services are available, by appointment, to uninsured Church Health patients and MEMPHIS Plan participants. You must have a referral from a physician, dentist, or nurse practitioner.
Your First Visit
- Physical therapy services are delivered on a sliding scale.
- Loose, comfortable clothes and sneakers are recommended.
- If you have a physician order or any relevant medical records, please bring them with you.
- Your first visit will last approximately 1.5 hours. Follow-up visits are typically 45—60 minutes.
Patient Access & Parking
There is parking on Concourse Avenue and in the Crosstown Concourse parking garage.
Please access the building via the Claybrook walk-in clinic entrance on the west side of the building. There is also drive-through access and drop-off at that location.
When you enter, take the elevator to the second floor Physical Rehabilitation waiting room for check-in.
