Update Yearly Eligibility Documentation

To be a patient, Church Health requires that your photo ID, proof of income and proof of address are current every year. You must bring in the documents listed below to continue to receive discounted services.

These documents can be sent through the patient portal, emailed to docs@churchhealth.org or faxed to 901-722-8078 (attention Medical Front Desk). Please be sure to include your name and date of birth when sending your documents.

Income Documents

  • 1040 form (we do not accept W-2 or 1099 form)
  • Checkstubs – require the most recent month’s worth (consecutive)
  • Letter from employer on company letterhead (must include hours and pay rate)
  • School schedule and financial aid letter/household income
  • Youngest child’s birth certificate and household income
  • Social Security Benefits Statement
  • Pension Letter
  • Household income
