Patient Instructions

We encourage you to receive the latest COVID vaccine. Please visit vaccine.gov to find a location near you.  Uninsured patients should select any location participating in the Bridge Access Program.

We also want you to be aware of your ability to order four free COVID tests per household from the government.  They can order online or get more information by clicking here.

All patients should check their Patient Portal frequently for updates. If you have problems logging in, please call 901-272-0003.
