Patient Instructions

During the pandemic, Church Health has modified clinic and appointment procedures to protect our patients, employees and volunteers from unnecessary risk of exposure to COVID-19.

Our Walk-In Clinic is open by appointment only! If you are sick or injured, you must call 901-272-0003 to make an appointment and to receive appointment instructions. If you do have an appointment, no more than one healthy person (child, parent, interpreter) should accompany a patient to an appointment, and then ONLY if you require personal assistance.

All patients should check their Patient Portal frequently for updates. If you have problems logging in, please call 901-272-0003.
