Obtaining Your Records

As an established patient, some of your records, such as your current medication list or visit summaries, can be found on your Patient Portal. Otherwise, you must come in person to fill out and sign a release of records form stating that you give permission to send out records. Once this form is completed, most records requests are usually handled within 710 business days.

About Insurance

If you are an established patient and purchase a plan from the Affordable Care Act marketplace exchange, Church Health currently accepts the following plans:

We also accept a variety of Medicare plans. Please contact us to see if we accept your plan.

Note: If you are NOT an established patient, we do not take insurance unless your carrier has assigned you to Church Health. For more information, please call 901-272-0003.
